Possible wellness activities provided by ACT are not a substitute for medical attention, examination, diagnosis, or treatment. Involvement in wellness activities may not be recommended under certain medical conditions.
By participating in any wellness activity, you agree to the following statement: I affirm that I alone am responsible for deciding whether to engage in any wellness activity. I hereby agree to irrevocably release and waive any claims that I have now or hereafter may have against ACT and any of its vendors in relation to the ACT Annual Meeting.
