MEMBERSHIP APPLICATION FORM

INSTRUCTIONS

Candidates for membership to the American College of Toxicology must comply with the following requirements. A copy of the supporting documents as requested on the application (including a detailed description of current and past positions) must be sent to the Secretariat of the American College of Toxicology. The application for Full membership must bear the names of two sponsoring Full members of the American College of Toxicology. The application for Associate membership must bear the name of one sponsoring Full College member. Current phone numbers of the sponsoring members must be provided on the application form.  For candidates employed overseas, letters of recommendation from two scientists/supervisors familiar with the candidate's work in toxicology will be accepted. For Student membership, a letter of recommendation from their academic advisor is required.

The Secretariat shall forward copies of the completed application with supporting information to the Membership Committee. The Membership Committee shall vote on the eligibility of the applicants for membership in the College. The Chairman of the Membership Committee shall compile the votes and forward the results to the Council of the College. Letters of acceptance or denial will then be sent to the applicants.
